About the 1.b question. I think Jon Batley did not do to much because he trusted on the effort of their employees. In this sense I am not very sure what the question ask.

I think you may have misunderstood the question. It isn't asking you what John Bailey actually did - it's asking you to come up with some ideas for what, if anything, he might have done that could have avoided / reduced the impact of the problems factors that you have highlighted in 1a.
